In order to move forward with the grant payment to your mortgage, you, and your co-applicant if applicable, must agree to the following and sign below:

• The information contained in the approval email is completely accurate.
• You will take responsibility to immediately contact your mortgage company and inform them of the grant payment amount and request that any surplus funds be applied to a suspension account toward future payments.
• You will take responsibility to confirm that the grant payment is applied to your account by the mortgage company within 20 days.
• You will contact us immediately if the payment is not applied to your account within 20 days, or if you receive notice that your mortgage has gone into foreclosure.
